Injury Liability Waiver & Medical Authorization Waiver

I, the undersigned parent or legal guardian, hereby grant my permission for my child(ren) to participate fully in any activities, practices, and games with Titans Fastpitch (“Titans Fastpitch authorized activities”).

I understand, recognize, and acknowledge that Titans Fastpitch authorized activities are potentially hazardous and may involve the risk of accident, death, illness, physical or mental injuries, and property damage. It is my responsibility to ask questions about any aspect of Titans Fastpitch authorized activities that has not been explained to my satisfaction. I hereby voluntarily assume any and all risks, including injury to person and property, related to my child(ren)'s participation in Titans Fastpitch authorized activities. I further understand that, notwithstanding precautions taken by Titans Fastpitch, sports and fitness activities involve a risk of injury and/or death. I, the undersigned parent or legal guardian, on behalf of my child(ren), are voluntarily participating in Titans Fastpitch authorized activities and using equipment with knowledge of the dangers involved.

I, the undersigned parent or legal guardian, hereby give my permission to take my child(ren) to a healthcare provider, doctor, or hospital and hereby authorize medical treatment, including but not limited to non-emergency or emergency medical treatment or surgery, and assume the responsibility of all medical costs, if any.

In consideration for my child(ren) being accepted for participation in Titans Fastpitch authorized activities, I hereby release, discharge, and agree to hold harmless Titans Fastpitch and its volunteers, directors, board members, employees, and representatives from any and all liability, claims, or demands for personal injury, sickness, death, damage, and expenses of any nature whatsoever which may be incurred by my child(ren) while participating in Titans Fastpitch’s authorized activities.

I, the undersigned parent or legal guardian, hereby assume the risk of personal injury, sickness, death, damage, and expense of any nature whatsoever which may be incurred as a result of my and my child(ren)’s participation in Titans Fastpitch's authorized activities.

I, the undersigned parent or legal guardian, hereby authorize and permit Titans Fastpitch and its volunteers, directors, board members, employees, and representatives to furnish transportation or call for transportation in the event of a medical emergency, if such is needed.

I, the undersigned parent of legal guardian, further agree to indemnify and hold harmless Titans Fastpitch and its volunteers, directors, board members, employees, and representatives from any liability sustained by Titans Fastpitch as the result of negligent, willful, or intentional acts committed by myself or my child(ren), including expenses incurred by Titans Fastpitch and its volunteers, directors, board members, employees, and representatives attending thereto.

Should it be necessary for my child(ren) to return home due to non-emergency medical reasons, disciplinary action, or otherwise, I, the undersigned parent of legal guardian, will provide the necessary transportation immediately after notification of such action or otherwise arrange for the necessary transportation immediately after notification of such action.
